All vendors must register with Green Security and complete the registration requirements before visiting a UAMS campus or clinic.
Register Online
To register as a vendor representative, go to www.greensecurityllc.com/register,
and click Vendor/Contractor Rep. The site will lead you through
a process to:
Provide your name and contact information
Select the UAMS facilities you want to access
Choose an access level:
Level 2B is only for administrative and nonpatient areas.
Level 3 is for patient care areas. It includes anywhere you may see patients receive care,
such as inpatient units, pharmacy, operating rooms and so forth.
When you complete registration, the site will generate an invoice for fees. If your company pays for
your credentials, a manager may need to log in and pay.
Provide required documentation
After you register, Green Security will contact you about submitting required documentation, which
depends on the level of access you requested. The levels and required documents are:
Level 2B – Non-clinical vendor representative
Background check
Acknowledgement of UAMS policies and procedures
Proof of product or service training
Proof of a TB or PPD skin test or, if positive, documentation of a chest X-ray
Proof of influenza vaccination
Level 3 – Clinical vendor representative
Background check
Acknowledgement of UAMS policies and procedures
Proof of product or service training
Proof of a TB or PPD skin test or, if positive, documentation of a chest X-ray
Proof of influenza vaccination
Proof of bloodborne pathogen training
Proof of hepatitis B vaccination or declination
Proof of MMR (measles, mumps, rubella) vaccinations
Proof of HIPPA/patient confidentiality training
Receive your badge in the mail
When you have cleared the background and sex offender registry checks, submitted all required
documentation and all payments are complete, Green Security will mail your badge via priority mail.
You will need to sign for the package.
Green Security badges are hard plastic and reusable rather than disposable.
