PURPOSE
To notify departments within the University of Arkansas for Medical Sciences (UAMS) of the procedures to be followed in the identification and removal from service of those items or equipment and furniture which may cause injury to patients, visitors, students and employees. During safety surveys and inspections, the Department of Occupational Health and Safety (OH&S) will red tag UAMS property that is deemed unsafe for its intended use. Each affected department is responsible for the actual removal from service of all red tagged items.
SCOPE
All UAMS employees, faculty and staff.
PROCEDURE
(1) The Department of Occupational Health and Safety shall red tag UAMS property found to be unsafe for intended use. Unsafe items shall be removed from service by the affected department and must not be placed back in service until repairs are made.
(2) The Department of Occupational Health and Safety will notify the immediate supervisor in the area when unsafe items have been red tagged. The supervisor will immediately remove the tagged item from service. The department director will then be notified that unsafe items have been red tagged and removed from service.
(3) The Department of Occupational Health and Safety will track the progress of repair of red tagged items through the use of the UAMS Hazard Report Form. This form will be completed by OH&S immediately upon discovery of unsafe items and routed to the appropriate department for corrective actions.
